ABSTRACT

Telemedicine, which integrates medicine, communication, engineering, information and other disciplines, is a hot emerging cross field in recent years. With the development of telecommunication technology and surgical robot, telesurgery is regarded as the "crown pearl" in telemedicine and has attracted more and more attention. As an extension of traditional surgery, telesurgery greatly extends the connotation and concept of surgery and embodies the great leap forward development of surgical technology. Despite the current limitations such as network delay, transparency of remote robot operation and team construction of surgeons, telesurgery has still formed a variety of innovative application scenarios and achieved rapid development in China in recent years. In view of the uneven distribution of medical resources in China and the epidemic of COVID-19 in the world, this paper puts forward the possible problems and solutions in the development of telesurgery, and looks forward to the feasibility of telesurgery technology in process of shifting the focus of medical and health care down to the community level, channeling resources accordingly.

ABSTRACT

Astragali Radix is one of the most commonly used medicinal materials. In recent years, its cultivated varieties and a variety of adulterants have flooded the market, which makes its quality uneven, and the development of quality control methods has become a research hotspot. Therefore, figuring out the quality markers of Astragali Radix is of great significance for its comprehensive evaluation. In this study, the fingerprints of 15 batches of Astragali Radix were established by HPLC, and the main components causing intergroup differences were screened out by PLS-DA. On the basis of literature review and network pharmacology analysis, the targets and pathways of active ingredients were obtained from SwissTargetPrediction, PubChem Compound and other databases, and then the "component-target-pathway" network was constructed with Cytoscape 3.7.1 for the prediction of potential quality markers. Twenty-eight common peaks were identified in the established fingerprint, and three differential components were selected as potential quality markers for Astragali Radix, which were astragaloside Ⅳ, calycosin-7-O-β-D-glucoside and ononin. The proposed method based on HPLC fingerprint of Astragali Radix is convenient and feasible, facilitating the improvement in its quality control.

ABSTRACT

italic>Bulbophyllum orchids are popular for its ornamental appearance and great medicinal values. However, there is still a lack of research on phylogenetic relationship and species identification for this genus. In this study, the plastome sequences of three medicinal Bulbophyllum orchids (Bulbophyllum affine, Bulbophyllum pectinatum, Bulbophyllum funingense) were sequenced and analyzed. After assembly and annotation, it was found that the plastomes of Bulbophyllum plants encoded a total of 108 genes, including 74 protein-coding genes, 30 tRNA genes and 4 rRNA genes. Based on the analysis of mVISTA and comparison between junctions, it was found that the plastome structure of Bulbophyllum orchids was relatively conserved, and the variation mainly existed in the non-coding regions. Phylogenetic analysis showed that Bulbophyllum orchids were closely related to Dendrobium orchids. SSR analysis of Bulbophyllum showed that most SSRs were located in the intergenic spacer and had the most single nucleotide repeats. In addition, based on the comparative analysis of non-coding sequences, a total of 10 high-variability sequences were screened out, among which the combination of five non-coding region sequences, including psbI-trnS, psbC-trnS, clpP-ex1-psbB, psaJ-rpl33, rpl33-rps18, had the highest sequence variability and could be used in the species identification study of medicinal plants of Bulbophyllum. In conclusion, this study provides a theoretical basis for phylogenetic relationship and species identification of Bulbophyllum orchids through the comparative analysis of plastome sequences of three medicinal plants of the genus Bulbophyllum.

ABSTRACT

PURPOSE: To compare the diagnostic efficiency of ⁶⁸Gallium labelled prostate-specific membrane antigen positron emission tomography (⁶⁸Ga-PSMA PET) and magnetic resonance imaging (MRI) for staging the lymph node metastases (LNMs) in the prostate cancer.MATERIALS AND METHODS: A broad search of scientific databases including PubMed, EMBASE, Web of Science, Cochrane Database, and Chinese Biomedicine Literature Database (updated prior to November 1st, 2018) was conducted systematically by two reviewers. In this paper, we evaluated the methodological quality of each included article independently and performed a systematic review and meta-analysis to reveal the summary of the diagnostic performance of ⁶⁸Ga-PSMA PET and MRI in properly identifying LNMs of intermediate- and/or high-risk prostate cancer.RESULTS: Thirteen eligible articles comprising 1,597 patients were included. For LNMs detection, the pooled sensitivity and specificity of ⁶⁸Ga-PSMA PET were 0.65 (95% confidence interval [CI]: 0.49–0.79) and 0.94 (95% CI: 0.88–0.97), respectively, while the corresponding values of MRI were 0.41 (95% CI: 0.26–0.57) and 0.92 (95% CI: 0.86–0.95). The area under the symmetric receiver-operating characteristic (SROC) curve for ⁶⁸Ga-PSMA PET and MRI were 0.92 and 0.83, respectively.CONCLUSIONS: In intermediate- or high-risk pre-treatment prostate cancer, ⁶⁸Ga-PSMA PET had a higher sensitivity and a slightly different specificity in probing the LNMs when comparing with MRI. Moreover, the area under the SROC curve indicated that ⁶⁸Ga-PSMA PET was a more effective weapon for predicting the LNMs prior to radical surgery.

ABSTRACT

Objective: To compare the effects of Hedysari Radix and Hedysari Radix Praeparata Cum Melle in Buzhong Yiqi and its chemical composition, and preliminarily reveal the mechanism of Hedysari Radix processed with honey. Methods: A rat model of spleen qideficiency was established. The rats were treated with different doses of water extracts of Hedysari Radix and Hedysari Radix Praeparata Cum Melle and the content of serum D-xylose, GAS, IL-2, TNF-α were used as indicators to study the differences in the efficacy of Hedysari Radix and Hedysari Radix Praeparata Cum Melle in Buzhong Yiqi. Based on HPLC techniques, the different components in methanol extract of Hedysari Radix and Hedysari Radix Praeparata Cum Melle were analyzed. Results: Compared with the blank group, the serum xylose, gastrin content in the model group were significantly decreased. Compared with the model group, except for the low dose group of Hedysari Radix, the serum xylose, and gastrin content of the rats in each administration group were significantly increased (P < 0.01), and the results in the middle dose group of Hedysari Radix were significantly higher than those in the middle dose group of Hedysari Radix Praeparata Cum Melle (P < 0.05, P < 0.01). Compared with the blank group, the serum IL-2 and TNF-α levels in the model group were significantly increased (P < 0.01). Compared with the model group, the serum IL-2 and TNF-α levels in the rats in each drug group were significantly decreased (P < 0.05, P < 0.01); Compared with the medium dose group of Hedysari Radix, the serum IL-2 and TNF-α levels in the medium dose group of honey-processing Hedysari Radix were significantly decreased (P < 0.05, P < 0.01). There were differences in the chromatographic peaks in the fingerprints of Hedysari Radix and Hedysari Radix Praeparata Cum Melle. Conclusion: Both Hedysari Radix and Hedysari Radix Praeparata Cum Melle can significantly intervene and improve the syndrome of spleen qi deficiency in rats and the pharmacodynamics effect of Hedysari Radix Praeparata Cum Melle is better than that of Hedysari Radix. There are differences in the components of Hedysari Radix and Hedysari Radix Praeparata Cum Melle and these differences may be the active substances that cause differences in the efficacy.

Objective To study the effect of unmovable splint therapy in patients with moderate and severe periodontitis. Methods Twenty periodontitis cases with loosened anterior teeth were treated with unmovable splints after the basic periodontal therapy. The clinical effect and periodontal index were evaluated in the sixth month, the first year, the second year and the third year. Result The success rate was 90 percent three years after the restoration.Conclusion Unmovable splint therapy is one of the effective therapies in the treatment of periodontitis.

ABSTRACT

In this study, TRAP molecular markers were used in identification of wild populations and hybrids of Dendrobium officinale, based on the sequences of genes encoding phosphoenolpyruvate carboxylase (PEPC), UDP-glucose pyrophosphorylase (UGP) and phenylalanine ammonia-lyase (PAL). Seven polymorphic target region amplification polymorphism (TRAP) primers were selected from 54 primer combinations and used in the identification of wild populations. Moreover, hybrids had female polymorphic bands, male polymorphic bands and heterozygous bands, which suggest that seven TRAP markers are able to identify the hybrids from their parents. Furthermore, the UPGMA dendrogram revealed that when sample from Guangnan in Yunnan province was used as one parent, reciprocal hybrids grouped with them in first, and then grouped with the other parent. The results indicated that the hybrids were closer to D. officinale from Guangnan population. This study identified the wild populations and hybrids of D. officinale by TRAP molecular markers, which is useful in selection of good varieties for artificial cultivation and early identification of hybrids. The study provides a method in the control of stability of germplasm and quality of D. officinale.

Bletilla striata has been used as traditional Chinese medicine for several centuries. In recent years, the quality and quantity of wild B. striata plants have declined sharply due to habitat deterioration and human over-exploitation. Therefore, it is of great urgency to evaluate and protect B. striata wild plant resource. In this study, sequence-related amplified polymorphism (SRAP) markers were applied to assess the level and pattern of genetic diversity in twelve populations of B. striata. The results showed a high level of genetic diversity (PPB = 90.48%, H = 0.349 4, I = 0.509 6) and moderate genetic differentiation among populations (Gst = 0.260 9). Based on the unweighted pair-group method with arithmetic average (UPGMA), twelve populations gathered in three clusters. The cluster 1 included four populations. There are Nanjing, Zhenjiang, Xuancheng and Hangzhou. The seven populations which come from Hubei Province, Hunan Province, Jiangxi Province and Guizhou Province belonged to the cluster 2. The cluster 3 only contained Wenshan population. Moreover, Mantel test revealed significant positive correlation between genetic distances and geographic distances (r = 0.632 9; P B. striata.

OBJECTIVE:To establish a method for the contents determination of 5 coumarins in sulphur-fumigated and no sul-phur-fumigated Glehnia littoralis,and investigate the effect of sulphur fumigation on the coumarins in G. littoralis. METHODS:HPLC was performed on the column of Hypersil ODS C18 with mobile phase of acetonitrile-0.1% phosphoric acid (gradient elu-tion)at a flow rate of 1.0 ml/min,detection wavelength was 295 nm,column temperature was 30 ℃,and the volume was 20 μl. RESULTS:The linear range was 0.121 5-12.15 μg/L for psoralen,0.389-38.9 μg/L for xanthotoxin,0.055-5.5 μg/L for bergapten, 0.070 5-7.05 μg/L for imperatorin and 0.092 5-9.25 μg/L for isoimperatorin(r≥0.999 7);RSD of precision test was lower than 1.0,RSDs of reproducibility and stability tests were lower than 3.0;recovery was 98.01%-101.97%(RSD<3.0%,n=6). CON-CLUSIONS:The content of coumarins in G. littoralis was obviously decreased after sulphur fumigation. the method is simple and accurate,and canbe used for the quality control of sulphur-fumigated and no sulphur-fumigated G. littoralis.

ABSTRACT

<p><b>OBJECTIVE</b>This review aimed to summarize the relationship between intestinal microbiota metabolism and cardiovascular disease (CVD) and to propose a novel CVD therapeutic target.</p><p><b>DATA SOURCES</b>This study was based on data obtained from PubMed and EMBASE up to June 30, 2015. Articles were selected using the following search terms: "Intestinal microbiota", "trimethylamine N-oxide (TMAO)", "trimethylamine (TMA)", "cardiovascular", and "atherosclerosis".</p><p><b>STUDY SELECTION</b>Studies were eligible if they present information on intestinal microbiota metabolism and atherosclerosis. Studies on TMA-containing nutrients were also included.</p><p><b>RESULTS</b>A new CVD risk factor, TMAO, was recently identified. It has been observed that several TMA-containing compounds may be catabolized by specific intestinal microbiota, resulting in TMA release. TMA is subsequently converted to TMAO in the liver. Several preliminary studies have linked TMAO to CVD, particularly atherosclerosis; however, the details of this relationship remain unclear.</p><p><b>CONCLUSIONS</b>Intestinal microbiota metabolism is associated with atherosclerosis and may represent a promising therapeutic target with respect to CVD management.</p>

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of scopolamine butylbromide in the adjuvant treatment of gastric ul-cer. METHODS:The data of 160 patients with gastric ulcer were retrospectively analyzed and randomly divided into tradition group and combination group. Tradition group was orally treated with acid suppression,hemostasis and other conventional treatment. Based on the treatment of tradition group,combination group was treated with Scopolamine butylbromide for injection 20 mg by in-tramuscular injection,once a day,for continuous 7 d,continue conventional treatment after 7 d. The treatment course was 8 weeks. The clinical efficacy,different complete relief patients with abdominal pain in different time,and the recurrence rate after 1 year in 2 groups were observed,and the incidence of adverse reactions were recorded. RESULTS:The total effective rate in combination group was significantly higher than tradition group,recurrence rate was significantly lower than tradition group,different complete relief patients with abdominal pain in different time were significantly better than tradition group,the difference was statistically sig-nificant(P<0.05). There were no obvious adverse reactions in 2 groups during treatment. CONCLUSIONS:Based on the conven-tional treatment,scopolamine butylbromide can be used in the adjuvant treatment of gastric ulcer. It can obviously shorten the time of abdominal pain in patients with gastric ulcer and reduce the recurrence rate,with good safety.

ABSTRACT

In this study, 17 kinds of Dendrobium species of Fengdous including 39 individuals were collected from 4 provinces. Mitochondrial gene sequences co I, nad 5, nad 1-intron 2 and chloroplast gene sequences rbcL, matK amd psbA-trnH were amplified from these materials, as well as nrDNA ITS. Furthermore, suitable sequences for identification of Dendrobium species of Fengdous were screened by K-2-P and P-distance. The results showed that during the mentioned 7 sequences, nrDNA ITS, nad 1-intron 2 and psbA-trnH which had a high degree of variability could be used to identify Dendrobium species of Fengdous. However, single fragment could not be used to distinguish D. moniliforme and D. huoshanense. Moreover, compared to other combined fragments, new type combined fragments nrDNA ITS+nad 1-intron 2 was more effective in identifying the original plants of Dendrobium species and could be used to identify D. huoshanense and D. moniliforme. Besides, according to the UPGMA tree constructed with nrDNA ITS+nad 1-intron 2, 3 inspected Dendrobium plants were identified as D. huoshanense, D. moniliforme and D. officinale, respectively. This study identified Dendrobium species of Fengdous by combined fragments nrDNA ITS+nad 1-intron 2 for the first time, which provided a more effective basis for identification of Dendrobium species. And this study will be helpful for regulating the market of Fengdous.

<p><b>OBJECTIVE</b>This study aimed to evaluate the effects of the microarc oxidation surface on cell morphology and cytoskeleton.</p><p><b>METHODS</b>Pure titanium with a diameter of 15 mm and a thickness of 1 mm was divided into four groups: grooved surfaces (group G); sandblasted surfaces (group SB); grooved microarc oxidation surfaces (group GMAO); and sandblasted microarc oxidation surfaces (group SBMAO). Osteoblast cells were cultured in each group. The morphology and proliferation of the cells on the titanium surface were observed by scanning electron microscope (SEM). The cytoskeleton was evaluated by laser scanning confocal microscope (LSCM).</p><p><b>RESULTS</b>The osteoblasts were inoculated after 12 h; these osteoblasts then spread along the surface of the titanium plate. In GMAO and SBMAO groups, the osteoblasts converged in the hole. The actin fibers in each group were clearly visible. In particular, the actin fibers in GMAO and SBMAO groups were arranged in parallel and formed bundles that extended into the holes.</p><p><b>CONCLUSION</b>After microarc oxidation, the titanium surface significantly affected the morphology and cytoskeleton of osteoblasts.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effects of repair of complicated tissue defects of several body parts with composite anterolateral femoral fascia lata perforator tissue flaps (fascial flap or fascial skin flap) with the aid of micro-surgery.</p><p><b>METHODS</b>From February 2008 to August 2012, complicated tissue defects in 12 patients were repaired with composite anterolateral femoral fascia lata perforator tissue flaps. Two of the 12 patients suffered from a defect of scalp, skull, and dura mater as a result of resection of a malignant tumor of the scalp; 3 patients showed a defect of skin and tendo calcaneus in the heel and lower leg; 2 patients showed a defect of skin and extensor tendon in the dorsum of hands; the other 5 patients suffered from defects of skin and extensor tendon in the foot and ankle combined with exposure of bone or internal buttress plate. The size of tissue flaps ranged from 12 cm ×6 cm to 19 cm ×18 cm. The donor sites were closed by immediate suturing or skin grafting.</p><p><b>RESULTS</b>All 12 tissue flaps survived. Patients were followed up for 2 to 36 months. The flaps were shown to have good appearance, texture and function. Two patients with the defect of the scalp, skull and dura mater after a resection of the malignant tumor of the scalp did not have recurrence or herniation of brain tissue. The foot-raising function in 3 patients with the defect of skin and tendo calcaneus in the heel and lower leg was recovered, and according to Arner-Lindholm criteria the result was excellent in 2 cases and good in 1 case. The extension function of fingers of 2 patients with defects of skin and extensor tendon in the dorsum of hands was good according to the evaluation criteria of Chinese Medical Association Society of Hand Surgery for tendon repair of hand. The extension function of toes of 5 patients with defects of skin and extensor tendon in the foot and ankle combined with exposure of bone or internal buttress plate was recovered and improved.</p><p><b>CONCLUSIONS</b>Transplantation of composite anterolateral femoral fascia lata perforator tissue flaps with the aid of micro-surgery is an effective method in repairing the tissue defects of skull, dura mater, and the extensor tendon of hands or feet, with restoration of the extension function.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the findings of computed tomography (CT) as objective markers of bone remodeling in rabbit models with chronic rhinosinusitis.</p><p><b>METHODS</b>Forty-eight rabbit models were established by vaccination of staphylococcus aureus. The rabbits were divided into 3 groups according to the time of infection: group A, B and C (4, 8, 12 weeks after infection), 16 rabbits in each group. Each group was subdivided into the medication administration team and the control team, 8 rabbits in each team. All the rabbits were examined by CT before vaccination to rule out the disease of nasal cavity and sinuses, and the CT images were used as the negative control. No interference was given to the control teams which were only examined again by CT when reached the end week. Dexamethasone sodium phosphate were administered to the medication administration teams 2 weeks before the end of experiment, and were examined by CT at the end. The images of both horizontal position and coronal position by reconstruction were obtained. The Hounsfield unit (Hu) of the bone which was the thickest position in each image were measured. The data was analyzed by SPSS 16.0 software. The Hu was analyzed statistically to compare the situation of the bone remodeling in different periods and administration in the rabbit models with CRS.</p><p><b>RESULTS</b>Average Hu (x±s) of normal rabbits was 810.0±99.7, average Hu at the end time: control team in group A was 964.0±84.6, medication administration team in group A was 833.0±92.5; control team in group B was 987.0±91.5, medication administration team in group B was 886.0±91.6; control team in group C was 1086.0±74.0, medication administration team in group C was 899.8±76.5. The Hu in all groups were higher than normal (t value were 2.747, 4.513 and 7.350 respectively, all P<0.05). No statistical difference was found between control teams of group A and B (t=0.423, P=0.667). The Hu in control team of group C was higher than group B (t=3.905, P=0.001); There was no statistical difference between medication administration teams of group A and B (t=0.892, P=0.384), and group B and C (t=0.886, P=0.385). The Hu of all medication administration teams in 3 groups were lower than all the control teams (t value were 2.717, 3.687, 8.379 respectively, all P<0.05).</p><p><b>CONCLUSIONS</b>Bone remodeling was found in rabbit models with rhinosinusitis, and the phenomenon was more obvious if the period was lengthened. The Hu could reflect the degree of bone remodelling. Glucocorticoids could depress the bone remodeling in the rabbit models with rhinosinusitis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate ideal methods to repair cervical cicatricial contracture in children.</p><p><b>METHODS</b>The expanders were implanted subcutaneously around the cervical scar and above the latissimus. After expansion was completed, the cervical cicatricial contracture was released and the wound was covered with local expanded flaps and free expanded prefabricated thoracodorsal artery perforator flap, leaving no injury to thoracodorsal nerves and latissimus. The wound at the donor site was closed directly.</p><p><b>RESULTS</b>From July 2007 to October 2009, 10 patients were treated. All the flaps survived completely. All the wounds were repaired totally and the deformities were corrected completely. The patients were followed up for 3-30 months. When the patients grew up, the flaps enlarged simultaneously. The flaps were not bulky and had a good color match. The scar at the donor site was inconspicuous with no functional morbidity.</p><p><b>CONCLUSION</b>The fabricated expanded thoracodorsal artery perforator flaps is an ideal method for severe cervical cicatricial contracture in children.</p>

@#Objective To investigate the expression of DR5 and DcR2 protein and mRNA in lumbar intervertebral disc (LID) of patients with prolapse of LID and normal controls.Methods The expression and distribution of DR5 and DcR2 protein were immunostained in prolapsed LIDs of 60 patients and 22 normal LIDs of 8 normal controls. In parallel, mRNA of DR5 and DcR2 was quantified by real time fluorescent reverse transcriptase-polymerase chain reaction (RT-PCR) in prolapsed LIDs of 30 patients and 9 normal LIDs of 3 normal controls.Results The positive rates of DR5 protein in prolapsed IVDs and normal IVDs were 41.60% and 26.09% respectively. There were a significant difference between the two groups ( P=0.001). Further similar evidences were obtained by quantification of DR5 mRNA ( P=0.025). There was no difference in the expression of DcR2 protein and mRNA between the two groups.Conclusion The apoptosis pathway induced by DR5/tumor necrotic factor-related apoptosis-inducing ligand (TRAIL) maybe exists in LID tissues.

ABSTRACT

<p><b>UNLABELLED</b>OBJECTIVE To study the effects of TiO2 blasted and acid-etched surfaces of cp-titanium on changing composition of oxide-film and attachment and proliferation of osteoblasts in vitro.</p><p><b>METHODS</b>cp-titanium discs were prepared and divided into 4 groups: TiO2 blasted (SB), sandBlasted and acid-etching (SLA1 and SLA2) and machine-polished surface (S1). Scanning electron microscopy (SEM), X-ray diffraction (XRD) and electron probe microanalysis (EPMA) were used to test surface morphology and composition of oxide-film. Osteoblasts were cultured on the titanium surface of 4 groups. MTT assay was used to measure the attachment and proliferation.</p><p><b>RESULTS</b>In regard to surface roughness, average roughness for SB, SLA1 and SLA2 was obviously higher than S0 Oxygen ratio increased on SB,contrarily, it decreased on SLA. A mixture of anatase and rutile-type crystals were observed in the SB. Smaller anatase were observed in the SLA1 and SLA2. The oxide thickness on SLA surface was thiner than that on the SB surface. Alter 1, 4, 24 hours' culture, the number of osteoblast attachment on SB surfaces was the highest (P < 0.05). The number of cells osteoblast proliferation was the highest on SB after 1, 3, 5 and 7 days' culture (P < 0.05).</p><p><b>CONCLUSION</b>The thickness and chemical composition of oxide film play an important role in osteoblast attachment and proliferation at the same roughness surface. It is concluded that osteobla.st attachment and proliferation are better on SB surfaces than on SLA1 and SLA2 surfaces.</p>
